RELEASE OF LIABILITY

I hereby give consent for participation in the above activity and claim that I (we) am in perfect physical condition to participate in said activity. I assume all risks and hazards incidental to such participation including transportation to & from the event and do hereby waive, release, absolve, indemnify and agree to hold harmless said person, association organizations, the organizer, sponsors, participants for any claim arising or any injury to myself or others in my group.
